Personal Information That We Collect
The personal information we collect depends on whether you are making an enquiry or commencing assessment/treatment with us.
When making an enquiry we request your name, contact details and a reason for contacting us. When commencing assessment/treatment we are required to gather and store more detailed information about you. This allows us to provide you with a safe and high-quality service.
We collect information about you that includes, but is not limited to:

- Date of birth
- Email address
- Telephone number(s)
- Home address
- GP surgery
- NHS number
- Occupational status
- Next of kin contact name and number
- Diagnosis (including date)
- Medical history including medications
- Current neurological symptoms
- What you are hoping to achieve from Physiotherapy
- Details of other health providers you may have recent or ongoing input from
- Financial details that relate to payment for our services (we do not store card details)
Why We Collect Your Data
Most of the personal information we process is provided to us directly by you (or a third party acting on your behalf) for one of the following reasons:
- To contact you following an enquiry
- For our own internal records
- To allow us to identify areas in which we should develop our service
We collect information from the following means:
- From telephone calls made to Neuro Physiotherapy Works

- Through our electronic ‘new client’ assessment form
- Face to face assessment and treatment sessions
- From other health professionals or care providers
To complete a safe assessment of you, we require accurate and detailed medical information. This information underpins the provision of care from us to you and is included in our legal documentation whereby we justify our assessments and interventions.
At times, we may be required to share your information with other health care practitioners, including but not exclusive to: GPs, Consultants, Physiotherapists, Occupational Therapists, Psychologists, Nurses, Speech and Language Therapists, Care Agencies, Case Managers. We will only share your personal data with your knowledge and consent with other appropriate professionals if applicable and appropriate. We will share information with other professionals e.g. GP and the police if you disclose any risk to yourself or others and this may be without your consent.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for collecting and processing your information are:
- Consent: By agreeing to this Privacy Policy you are consenting to us processing your personal data. You are able to remove your consent at any time using the contact details provided. However, if you object to the collection, sharing and use of your personal data we may be unable to provide you with our services.
- Legal Obligation: As professionals, Chartered Physiotherapists have a legal obligation to keep an accurate record of contact with clients and therefore personal information is required in order to complete documentation fully.
- Legitimate Interest: We have a legitimate interest to collect your data to enable us to do our job effectively and safely. We also have a legitimate interest in good governance and auditing business operations
Storing Your Data
We will only keep your information for as long as is deemed necessary. If you have contacted us for details of our services, and where this does not progress to assessment or treatment and we do not have any contracts with you, we will retain your personal data for a period of 12 months. If you receive assessment or treatment, we will retain your personal data for 8 years following the completion of intervention. This data will be retained for the protection of you and the service and is required by professional liability insurance Terms and Conditions. Sometimes we may need to retain your data for longer, for example if a complaint has been made or we are defending ourselves in a legal dispute.
We store personal information on secure devices and occasionally hard copy files that are kept in a secure location. Wherever possible, we limit paper copies of information. Personal information that we store or is protected by security controls, including two-factor authentication, and data encryption where appropriate.
All data that is no longer required is securely deleted or destroyed.
Your Rights Under GDPR
Under data protection law, you have rights including:
